Maddison mounted a brilliant comeback against Keinan Davis of Aston Villa in the semi-final earlier in the day, recovering from 3-1 down to win 4-3 via golden goal.
Wilfred Ndidi, who represented the Club in the inaugural tournament last month, headed home in the second minute of injury time for 3-3 before Jamie Vardy was on target to in extra-time complete a stunning turnaround.
Having defeated Callum Wilson, Max Meyer, Michael Obafemi and Davis en route to the final, Maddison took on Blades defender Egan in the final.
The No.10 raced into a 2-0 lead thanks to a first-half brace from Vardy, before Kelechi Iheanacho also struck twice in the second period.
Maddison, who scored with himself for 5-0 before Egan pulled a goal back, said after the game: "It was one of my best performances, to be fair. I put Wilfred Ndidi in to cut passing lanes.
"It was great fun and John's been a great contestant, but there was only one winner there!
"Jamie Vardy scores goals. Fact. He scored at Bramall Lane and scored a couple today. Maddison and Vardy get on quite well on FIFA!
"Seeing the first tournament, it's been fantastic and the fan engagement on social media has been brilliant."
